Crisis Intervention
Our Crisis Case Managers meet face-to-face with consumers to develop a plan that provides Community Catalysts of California with detailed instruction on how to offer mental health support, emergency support and relief services whenever our consumers are in need. Crisis Response Assistants also provide additional onsite support as needed.
Our on-call staff is ready 24-hours a day, seven days a week. We realize emergencies don’t always happen during business hours.
Our Triage Crisis Home is a State licensed Community Care Facility designed to serve adults who are in severe crisis, such that they are in jeopardy of losing their community placements and being sent to more restrictive environments. During their ninety day stay, our staff works intensely with psychiatric and psychological consultants, regional center personnel, and other key people in the consumer’s lives in an attempt to teach the individuals the appropriate skills needed to maintain a less restrictive community living placement.
Each consumer is assessed and a service plan is developed. The outcome is to help the individual and/or his/her circle of support learn new techniques and skills in dealing with the presenting issues, thus, reducing the behaviors to an acceptable level for community placement or a similar one being the overall goal.
Expected Consumer Outcomes:

Each individual (to the best of his/her abilities) and/or his/her circle of support will participate in the setting of personal goals and objectives within the program

Each individual will acquire or improve measurable skills for avoiding crisis situations

Each individual’s level of competency and self control will increase, so that the overall level of support and intervention will decrease

The number of individual’s needing placement in a more restrictive setting will measurably decrease
The Rapid Emergency Service Team (R.E.S.T.) program is designed to provide short-term crisis services to help assist in integrating and stabilizing consumers in the least restrictive environment possible in the community. This response is provided on a 24-hour on-call basis and the services provided vary upon the circumstances of the situation. If consumers or staff needs help, our staff are contacted and extra help is on the way. This service has been extended to consumers working in other agencies that lack the personnel resources or trained professionals. In all cases, the on-duty professional has necessary information for quick access regarding a consumer enrolled in the service.
