Home-Based Services
Home-Based Services are geared towards assisting individuals in living as independently as possible in their own communities. Our staff team provides Case Management, Personal Support, Counseling, Individual Instruction (safety, budgeting, etc.), Self Advocacy and Recreational support.
We have five different programs which are: Supported Living, Independent Living, Social & Recreational, Respite Care, and Licensed Residential Services.
Supported Living
In the supported living program a consumer may be paired with a roommate, neighbor, or other support person. This roommate may share the living space, the household responsibilities and expenses, or just be close by when needed to provide added security and companionship. Additional staff called, coaches and in-home support service workers may provide additional services as needed.
Supported living is an alternative to halfway houses or pre-apartment group homes. Individuals who would not have been expected to function independently find themselves able to cope with daily living problems with the added support so they can live in their own home or apartment.

Independent Living Services
Community Catalysts of California offers assistance and training to help consumers with the skills necessary to have one's own home or apartment with minimal assistance to be as fully self-sufficient as possible. The process begins by increasing self-reliance by means of providing a balance of support, counseling and training in the home.
In independent living a staff will come out to a consumer’s apartment or home usually a few hours each week as needed to help the consumers be successful in maintaining their living situation.

Social and Recreational Services:
These services are for those individuals who would like to enhance the quality of their life by participating in social activities in their community. Community Catalysts of California facilitates the involvement of people with disabilities in a variety of activities in the community by providing necessary support to the consumers to identify, organize and attend activities.
The psychological benefits of participating in social activities are well documented. If an individual finds that participation in social and recreational activities is still possible despite a congenital or acquired disability, then self-motivation often leads to other accomplishments. Issues of family, independent living, employment, and community participation are frequently addressed with a renewed perspective. Participating in activities can assist individuals in coping with stress, avoiding depression or trouble with alcohol or drugs.

Respite Care:
Our respite care provides temporary care for persons with developmental disabilities by our trained providers. Our program was developed to provide an opportunity for family members to take a break from caring for their family members with developmental disabilities. Whether it be a weekend rest, a week vacation or just a day to oneself, we can help. And we are always there during emergencies. Our care is usually provided in either the provider’s home or in the home of the family requesting assistance. Top of Page
Licensed Residential Services:
Licensed residential services are provided through State licensed Community Care Facilities in the community. These facilities provide 24 hour, 7 days a week support and supervision to adults with disabilities. These homes are an option for placement for individuals with developmental, physical and emotional disabilities who are in need of more structured and supervised services. This service allows individuals who may otherwise be limited in housing opportunities to remain in the community with a sufficient support system in place so they can be successful.
We also provide short-term, crisis services through licensed residential facilities. These short-term ciris facilities are designed to serve adults who are in severe crisis, such that they are in jeopardy of losing their community placements and being sent to more restrictive environments. During their ninety day stay in these types of facilities, our staff works intensely with sychiatric and psychological consultants, regional center personnel, and other key people in the consumer's lives in an attempt to teach the individuals the appropriate skills needed to maintain their less restrictive community living placements.
